WAC 10-08-210
Adjudicative proceedings -- Initial or final
order. Every decision and order, whether initial or final,
shall:
(1) Be correctly captioned as to the name of the agency and
name of the proceeding;
(2) Designate all parties and representatives participating
in the proceeding;
(3) Contain appropriate numbered findings of fact meeting
the requirements in RCW 34.05.461;
(4) Contain appropriate numbered conclusions of law,
including citations of statutes and rules relied upon;
(5) Contain an initial or final order disposing of all
contested issues;
(6) Contain a statement describing the available
post-hearing remedies.
