Commercial feed license application
requirements. (1) The commercial feed license application
form, to be completed by applicants and licensees, must
include:
(a) The company name and mailing address of the
applicant;
(b) The physical address of the facility;
(c) The name, contact information, and signature of the
applicant;
(d) Information regarding the types of business the firm
is engaged in (feed manufacturer, dealer, broker); and
(e) The type of commercial feed distributed (medicated
feed, complete feed, feed supplement, or animal by-products).
(2) A commercial feed license is not required for
facilities that only:
(a) Sell food processing by-products from fruit,
vegetable, or potato processing plants, freezing or
dehydrating facilities, or juice or jelly preserving plants;
(b) Sell bona fide experimental feed on which accurate
records and experimental programs are maintained;
(c) Makes retail sales of bagged, or packaged commercial
feed bearing labeling or other approved indicators showing
that the commercial feed is from a licensed manufacturer,
guarantor, or distributor who has assumed full responsibility
for the required tonnage inspection fees.
