Evaluation and issuance of certificate. (1) After preliminary requirements (WAC 173-50-060 through173-50-080
) have been met, the lab accreditation unit submits
a report to the affected laboratory concerning the results of
the overall accreditation process. The report:
• Lists findings;
• Assesses the importance of each finding; and
• Makes recommendations concerning actions necessary to
assure resolution of problems.
(2) After completing the accreditation review, the
ecology accrediting authority decides whether accreditation
should be granted.
(a) If accreditation is warranted, the department issues
a certificate and accompanying scope of accreditation. The
certificate remains the property of the department and must be
surrendered to the department upon revocation of accreditation
status.
(b) If accreditation is not warranted, the department
issues a report specifying areas of deficiency and steps
necessary to upgrade the laboratory to accredited status. In
such cases, the laboratory must provide documentation that the
specified deficiencies have been corrected. Based on such
documentation the ecology accrediting authority decides
whether to grant or deny accreditation.
