(1) Adequate toilet facilities must be provided for all
employees, and for each sex. Facilities must be conveniently
located and accessible, and must be maintained at all times in a
usable and sanitary condition and in a condition which is not
offensive to a reasonable person.
(2) The following table must be used as a guide in
determining the adequacy of toilet facilities.

(3) Whenever urinals are provided, one urinal may be
substituted for one toilet, provided the number of toilets must
not be reduced to less than two-thirds of the number shown in the
foregoing table.
